    • Name*
    • Email*
    • Priority (select from dropdown, default is "Low")
    • Subject*

Step 3: Complete Your Support Request

When filling out the support request form:

  1. In the "Subject" field, type "LMS Account Reset Request"
  2. In the "Message" field, include:
    • Your full name as registered in the system
    • Your Staff/Student ID number
    • The email address registered with your account
    • A brief description of the issue you're experiencing
  3. Set the "Priority" according to your urgency (Low, Medium, or High)
  4. Attach any relevant screenshots if they help illustrate the issue
  5. Complete the SPAM prevention check and submit your ticket

Step 4: Verification

The ICT team will verify your identity to ensure the security of your account. This may involve confirming your details against the school records or asking you to present your school ID in person.
